When creating care plans for a Care Recipient, Birdie offers assessment recommendations. These suggestions help you customize and address specific aspects of the Care Recipient's needs, ensuring their well-being and safety with comprehensive care and support.

This table provides a clear overview of each assessment type and the rationale behind its recommendation in the care plan.

Assessment Type

Logic/Explanation

Medication Assessment

If you support the Care Recipient with medication management, this assessment is recommended. This ensures proper understanding, administration, and monitoring of medications.

Waterlow Assessment

If the Care Recipient has a history of pressure sores, this assessment is recommended to assess the risk of developing pressure ulcers and to implement preventive measures.

Mental Capacity Assessment

If the Care Recipient lacks or may lack the capacity to make decisions related to health and welfare, this assessment is recommended. This ensures appropriate decision-making processes and safeguards for the Care Recipient's well-being.

Environment and Fire Risk Assessment

If the Care Recipient has concerns about their home environment or engages in behaviours such as smoking, this assessment is recommended. This ensures the safety and security of the Care Recipient in their living environment.

Moving and Handling Assessment

If the Care Recipient is not independent with mobility, transfers, or navigating stairs, this assessment is recommended. This helps in devising strategies to assist the Care Recipient safely and comfortably in daily activities.

Communication Assessment

If the Care Recipient experiences impairments in vision, hearing, or speech, this assessment is recommended. This facilitates effective communication and ensures the Care Recipient's needs are understood and met appropriately.

Dysphagia Assessment

If the Care Recipient has impaired swallowing, this assessment is recommended. This assesses the risk of choking and aspiration and provides guidance on safe eating and drinking strategies.

Financial Assessment

If the Care Recipient is unable to manage their finances independently, this assessment is recommended. This ensures appropriate financial management and safeguards against financial exploitation or mismanagement.
