How to report an incident or accident through the Birdie app

Birdie’s Raise a Concern flow has been updated to include an easy-to-use design - and ensures you are capturing critical information needed for high-quality reporting.

Raising a concern is an instant way to notify your Care Manager of any accidents, incidents or other concerns that take place during your visit. You have a duty of care to ensure your care recipients' safety by raising an alert immediately and by providing as much detail as possible.

When raising a concern within the Birdie app, you have the option to select whether that individual concern is made ‘public’ (meaning Care Managers, Family Members and other Care Professionals will see this concern) or ‘private’ (meaning only Care Managers will be able to see this concern).

Your Care Manager will instantly receive a notification on the agency hub with details of the alert you have raised.

What kind of concerns can you raise?

  • Accident - an event that results in injury or ill health

  • Incident - an event not causing harm, but has the potential to cause injury or ill health

  • Skin integrity - The skin is damaged, vulnerable to injury or unable to heal normally (e.g a pressure sore)

  • Medication - an event where the expected course of events in the support and/ or administration of medications is not followed

  • Other - any other event that will need to be recorded such as an issue with the house


To raise a concern, click the Raise a concern button in the top right-hand corner of your Birdie app - please note, you will need to be checked into a visit first.

Depending on what kind of concern you have selected, you will be prompted to fill out information relevant to that type of concern. Some concerns require different or additional information.

How to raise an 'Accident' or 'Incident' concern

  • Click raise a concern

  • Select the concern type (accident or incident)

  • Select the type of concern (i.e. behaviour, slip/fall, etc) and document what could have caused the event to happen

  • Describe the event

  • Describe the impact this had on the care recipient (add optional body map for further detail)

  • Document the Actions taken

  • Select the Severity

  • Select the Privacy

  • Identify the people involved in the concern (one or multiple - including care professional, care recipient, and/or other)

  • Document the date and time of the incident - select the date of the incident using the calendar, then select the time using the scroll menu

  • Identify the location of the concern

  • Click submit

Please note: questions that have a red star next to them are mandatory fields - you will not be able to continue and click the next arrow until complete/filled in.


How to raise a 'Skin integrity' or 'Medication' concern

  • Click raise a concern

  • Select the concern type (skin integrity or medication)

  • Describe what happened using the free-text box

  • Add a body map to identify where the concern is related to the care recipient

  • Document the Actions taken

  • Select the Severity

  • Select the Privacy

  • Click submit

How to raise an 'Other' concern

  • Click raise a concern

  • Select the concern type (other)

  • Add a title of the concern, using free-text

  • Describe what happened

  • Document the Actions taken

  • Select the Severity

  • Select the Privacy

  • Click submit
