What is Care Planning?

Care Planning is one category that contributes to the overall Birdie Q Score. It illustrates how effective, responsive, and person-centred your care is.

These are the 3 metrics we look at to assess your performance:


1. Percentage of clients with more than 3 tasks scheduled.

2. Percentage of tasks scheduled with notes.

3. The average number of days since the tasks were updated.

To increase your Care Planning Score, you can:

1. Ensure that all clients have more than 3 tasks scheduled.

2. Add notes to all scheduled tasks.

3. Regularly update your client's tasks, so that the average number of days since the tasks were updated stays low.

How do we help you improve?

In addition to your monthly Q-Score report, every Monday evening we will send a set of weekly reports to help keep you on track. There are five reports each providing a different view of care delivery and care management. Care Planning and The Birdie Q Score:


Here is how the Q Score is scored:

1: Inadequate

Less than 70% of clients have more than 3 tasks in their care plan

Less than 66% of scheduled tasks have notes

More than 100 days since the care task plan has been updated

2: Requires Improvement

More than 70% of clients have more than 3 tasks in their care plan

Less than 66% of scheduled tasks have notes

More than 100 days since the care task plan has been updated

3: Good

More than 80% of clients have more than 3 tasks in their care plan

Less than 66% of scheduled tasks have notes

More than 100 days since the care task plan has been updated

4: Outstanding

More than 90% of clients have more than 3 tasks in their care plan

More than 66% of scheduled tasks have notes

Less than 100 days since the care task plan has been updated


What to do if more than 70% of your clients have less than 3 scheduled tasks in their care plans:

Look at your Care Management by Client weekly report. This will give you an overview of how many of your clients need more tasks. We believe a varied task plan will have more than 3 tasks and will include tasks covering each different area of care.

From looking at my report below I can see that I should start my work with Annie, Anju, and Damian.

What to do if less than 66% of scheduled tasks have notes:

Referring back to your Care Management by Client weekly report. You will be able to see the percentage of tasks with notes per client.

Adding notes to tasks shows you are actively making your care as person-centred as possible. It also shows you are doing that extra bit in making sure your clients get the care they deserve.

What to do if it has been more than 100 days since your client's care plan has been updated:

Referring back to your Care Management by Client weekly report. You will be able to see the average number of days since any task has been updated. We believe a detailed, varied task plan will have some changes.

Even if the change is so small, it is still showing you are communicating with your clients and acting responsively to their needs. Reviewing this report will make sure you are keeping on top of all updates. It is also a very handy report to show to the CQC!
